What to expect
At your first visit our office staff will make the patient intake process as easy and painless as possible for you. We ask that you arrive 10 minutes early wearing comfortable, loose fitting clothes that will allow you to move freely. As a courtesy to you we will verify your insurance coverage and discuss your benefits before you get started with your initial evaluation.
At your first appointment, you will need to bring:
- New patient paperwork, or this can be filled out in office
- Physical therapy prescription
- Insurance card
- Any X-Ray, MRI, Doctor Notes, or Surgical Reports that will allow us to get a complete picture of your situation
The initial evaluation will begin with a conversation between you and the therapist. This conversation, along with tests and measurements, will help the therapist diagnose the cause of your pain or dysfunction, develop a treatment plan, and set goals. At this time we will also view your reports and answer any questions you may have about your injury, procedure, or the physical therapy process. From here, we will start you treament and the journey of making your body the best it can be.
After your initial evaluation, the frequency and duration of your visits will be discussed. PLEASE be sure to book your appointments 4 to 6 weeks out to ensure the times and days that are convenient for you are available. If not, we cannot guarantee that the appointment times you prefer will always be available to you.
Here at Synergy we structure our schedule so that each patient gets personalized treatment. Because of this, it is important that you show up on time to your appointment. If you are going to be late please call the office, and note that we may ask you to reschedule based on our schedule for that day. We do understand that life happens, if you are unable to make your scheduled appointment please call at least 24 hours in advance to avoid the NO SHOW and LATE CANCEL fee of $25 (insurance will not pay this fee).
